Providers Required To Update Medicare 855 Form On An Ongoing Basis

Many providers and suppliers have become familiar with the complex and often frustrating form known as the 855, but formally called the Medicare and Other Federal Health Care Program General Enrollment, Health Care Provider/Supplier Application. Since 1997, the Health Care Financing Administration (“HCFA”) has required all new providers and suppliers seeking reimbursement under Medicare to submit the 855 Application to enroll in the program. Although many providers and suppliers are savvy regarding their obligation to notify HCFA when they experience  significant changes, such as changes of ownership, most are unaware of their continuing obligations to update the information furnished through the initial enrollment process.
